Description:

Assisting a patient with bathing, dressing, toileting, grooming, eating, physical transfer and those normal daily routines that the patient could perform himself were he physically able. Keep records of the personal health care activities provided. Each month each case is evaluated by a RN to assess the patient's condition and the quality of care being provided. Observe the appearance and gross behavioral changes in the patient and report these changes to the family and the nurse registry or the RN responsible for asessing the case.
